TREATMENT WORKS APPROVAL SUBMITTAL REQUIREMENTS

*** All TWA Forms can be found on the New Jersey Department of Environmental Protection (NJDEP) website ***

1. Completed Treatment Works Approval Permit Application (TWA-1)
2. Engineer’s Report for Domestic Treatment Works Approval Application (WQM-006)
3. Statements of Consent (WQM 003-T) with signature certifications A, B, and C
   

   NOTE: There are municipalities within the MCUA Service Area that wastewater is conveyed through more than one municipal and/or Authority conveyance system. The WQM 003-T form must be signed by each owner/operator of that sewer the wastewater will be conveyed through prior to entering the MCUA Trunk System.

   2. Utility Plan
   3. Sanitary Sewer Detail
   4. Sanitary Sewer Profile
      
      
5. TWA Application Review Fee
   Upon receipt of the above submittal requirements, the MCUA will issue an invoice for payment of the TWA application review fee. The fees for sewer extensions can be found in Section 14 of the MCUA Rules and Regulations.
